The following downloadable forms exist: - Citizen Supplemental Information Report You can print these forms out, fill them in, and mail them to OPD to report a crime. Do NOT use these forms for crimes in progress! Use these to report committed crimes where the criminal has left the area. The link below leads to the OPD's recommended Suspect Description Form, which you can use to remind yourself what information OPD would like to have about a suspicious person or automobile. |
